The duly filled in application should be sent with a crossed D.D for
Rs. 500/- payable to ‘Shree Devi Institute of Technology’ drawn on a bank
at Mangalore. Registration fees once paid will not be refunded. The following
documents should be enclosed with the application:

1. One photocopy of the Statement of Marks of the 10+2 qualifying examination.
2. One photocopy of the S.S.L.C. Certificate showing the date of birth.
3. One photocopy of Transfer Certificate/School Leaving Certificate.
4. One photocopy of the Migration Certificate.
5. Three passport size photographs of the candidate.
6. One photocopy of the Statement of Marks of the Degree qualifying examination for MBA and MCA
